Post High School Placement Report for students enrolled in Perkins funded NYSED-approved CTE programs who reached the concentrator level and left secondary education any time during the 2024-25 school year due to the Office of CTE. |

First-time Applicants: Both first time and returning applicants that did not apply for funding in the 2025-26 program year, with an individual allocation of or greater than $15,000 and whose programs meet the Perkins V definitions of size, scope, and quality as verified in the Student Information Repository (SIRS – 305 report) at the time of application, must submit a letter of intent to apply by March 15, 2026, to both the fiscal agent of the consortium joined in the prior year and to the Office of CTE. A PDF copy of the original letter should be sent to the Office of CTE. A completed CLNA is then due to the Office CTE by April 1, 2026. Returning Applicants: |

Email one electronic copy of the 2026-27 Perkins Application (in Word format), transmittal letter, and FS-10 budget (in Excel format) to the Office of CTE. Final Narratives for 2025-26 Perkins V program year due to Office of CTE. Email an electronic copy of the Final Narrative (in Word) to the Office of CTE and send the hard copy Final Narrative with original signatures to: Office of CTE |

Complete applications for re-approval of CTE programs to continue to be offered in the 2026-27 school year due to the Office of CTE. Please note incomplete applications will be returned. Any issues identified by the Office of CTE must be resolved immediately. Failure to receive program re-approval from the Office of CTE by September 30, 2026, will result in a lapse in the program. Lapsed programs will be removed from the NYSED-approved list and in NYSED’s Information Reporting Services’ Level 2 data, resulting in student CTE data being blocked from SIRS reporting. These lapsed programs are not considered to be active, and therefore, they are not eligible to award the 4+1 CTE Pathway, integrated/specialized academic credit, and the technical endorsement beyond the 2025-26 school year. |
| 8/31/26 | Re-registration for work-based learning programs expiring 8/31/26 due in the NYSED Business Portal (SED Monitoring and Vendor Performance System). |

FS-10-F Final Expenditure Report for 2025-26 Perkins V program year due to the NYSED Office of Grants Finance. Send one FS-10-F Final Expenditure Report with original signature and one copy to: Grants Finance NEW: Email an electronic copy of the Final Expenditure Report for 2025-26 to the Office of CTE. |

Complete applications for initial approval of new programs to be offered in the 2027-28 school year due to the Office of CTE. Please note that all applications that are incomplete will be returned to the applicant and will not be considered submitted for deadline purposes. Any issues identified by the Office of CTE must be resolved immediately. If program approval is not obtained by December 1, 2026, students will be ineligible to enroll in a NYSED-approved program for the fall of 2027, and student data cannot be reported to the Student Information Repository System (SIRS) for the 2027-28 school year. |

A pre-populated Post High School Placement Report will be sent to each approved-program provider by mid-October 2026. If you do not receive your report, please contact the Office of CTE. |
