Register a Program to Prepare Candidates for State Teacher, Educational Leader, or Pupil Personnel Services Certification

Note: these forms are limited to proposals for programs designed to prepare candidates for State teacher, educational leader, or pupil personnel services certification. SUNY and CUNY institutions should consult with their respective system administration contacts.

Revised Student Teaching and Field Experience Regulations

At their July 2014 meeting, the Board of Regents adopted revisions to Commissioner’s Regulations section 52.21(b) governing field experience, student teaching, and practica required of teacher preparation programs in New York State.  (see Regents Item 714brca ) The Regents amended regulations to provide an option for a single student teaching placement (or practica), effective July 30, 2014, for teacher preparation programs leading to most certification areas.  For more information go to Revised Student Teaching and Field Experience Regulations.


Submission of Applications

1. Create a single PDF document that includes the following completed forms:

  • Appropriate application form (see options under " Application Materials" in the chart below)
  • Supplement(s) to the application (as required)
  • Signature document

2. Create a separate PDF document for any required syllabi or CVs

3. Attach the PDF documents to an e-mail.

4. Send the e-mail to OCUEedapps@nysed.gov with the Subject: Institution Name, Degree Award, and Program Title.  Example: AAA College, New Program, Master of Arts in Teaching, Adolescent Education

Application Materials

New: for Institutions that Hold an Absolute Charter or Permanent Degree Authority

Propose a New Teacher Certification Program:

  • Preparing candidates for Initial certification
  • Preparing candidates for Initial/Professional certification

Please note: this does not include Transitional B or C programs.

Required:

  1. Application form: New Teacher I/IP-Attest  

 

Supplements as needed:

  • general and program specific requirements

  • add distance education format
  • master plan amendment
  • external review report and response

Propose a New Teacher Certification Program (institutions with provisional charter or provisional degree authority) :

  • Preparing candidates for Initial certification
  • Preparing candidates for Initial/Professional certification

Please note: this does not include Transitional B or C programs.

Required:

  1. Application form: New Teacher I/IP 
  2. Signature document

Propose a New Teacher Certification program - Professional or Transitional:

  • Preparing candidates for Professional certification
  • Preparing candidates for Transitional B (alternative) certification
  • Preparing candidates for Transitional C (alternative) Certification

  1. Application form: New Teacher--Professional Certification, Transitional B, Transitional C 
  2. Signature document

Propose a New Educational Leader Program:

  • Preparing candidates for initial/professional or professional certification
  • Preparing candidates for Transitional D certification (alternative route: School District Leader)

  1. Application form: Educational Leader and Transitional D 
  2. Signature document

Propose a New School Psychologist or PPS Bilingual Education Extension Program:

  • Preparing candidates for School Psychologist certification
  • Preparing  PPS candidates or certificate holders for the Bilingual Education Extension

  1. Application form for: School Psychologist or the PPS Bilingual Education Extension  (124 KB)

Propose a new School Counselor Program

The Department is not accepting proposals for new School Counseling Programs at this time. We are developing a new application to meet the new School Counseling Program registration regulations and will post the application soon. For more information on the new regulations see the decision item presented to the Board of Regents in May 2017.

Distance Education

Colleges and universities located in or operating in New York State that offers degree or certificate programs in which a major portion (i.e., 50 percent or more) of the requirements can be completed through study delivered by distance education must have those programs registered in the distance education format.

Master Plan Amendment (MPA)

When an institution seeks to expand its academic mission (e.g., by offering a degree at a new level of study or in a new disciplinary area), it must submit a master plan amendment application for review and approval by the Board of Regents.

External Review Report

An external review of the proposed program is required if the program is a graduate degree program; or if the program's subject matter represents a new or emerging field; or if the external review is requested by the Department.
