Program Registration Applications, Guidelines, and Resources

Update: the Department’s approval of external reviewers for most proposals is no longer required prior to the reviews being conducted. For full details, please see our external review guidance page.

This page contains links to the forms needed to register a new program below the doctoral level or to change an existing program. An institution considering a new doctoral program should contact the Office of College and University Evaluation at (518) 474-1551 for the document, "Steps in the Review of Doctoral Program Proposals."

  • Please use the links below to access the application materials by general type of program proposal.
  • Please follow the instructions for submitting proposals, as described in the application materials. We encourage you to submit program proposals through e-mail unless otherwise noted. 
  • SUNY and CUNY institutions should consult with their respective system administration contacts. 

There are three primary types of applications for programs below the doctoral level. Please choose the application type according to the nature of the proposed program:

  1. Program to prepare candidates for teacher, educational leader, or pupil personnel services certification

  2. Program to prepare candidates for a professional license

  3. All other (general academic) programs

NEW: To request ONLY a title change for a registered program - 

Note: Completed title change request forms must be submitted to either the Office of College and University Evaluation (OCUE) or the Office of the Professions (OP), depending on which office registered the program. To identify the office that registered the program, refer to the Unit Code field on the program’s listing in the Inventory of Registered Programs.
