New FP-AU (Application for Use) Form

To better align with our current submission requirements, the FP-AU form has been replaced by two new forms:

• FP-AU (INSTR): Application for Use - Instructional Building
• FP-AU (NON-INSTR): Application for Use - Non-Instructional Building

Please use the form that corresponds to the building’s intended use when submitting requests for new leases, building acquisitions, or any other situations involving the use of space that has not yet been approved by NYSED. The old FP-AU form will no longer be accepted as of July 1, 2025.
