Regents Actions in 22 Professional Discipline Cases and 1 Restoration Petition

The Board of Regents announced disciplinary actions resulting in the revocation of 3 licenses, surrender of 6 licenses, and 13 other disciplinary actions. The penalty indicated for each case relates solely to the misconduct set forth in that particular case. In addition, the Board acted upon 1 restoration petition. 

I. REVOCATIONS AND SURRENDERS

Chiropractic

James Paul Walzer; Chiropractor; Fayetteville, NY 13066; Lic. No. 008622; Cal. No. 31934; Found guilty of professional misconduct; Penalty:  Revocation.

James Robert Graziano; Chiropractor; Central Islip, NY 11722-3822; Lic. No. 010072; Cal. No. 33347; Application to surrender license granted.  Summary: Licensee admitted to the charge of having been convicted of Driving While Intoxicated, as an unclassified misdemeanor, and Driving While Intoxicated, as an E felony.

Nursing

No. 33371; Application to surrender license granted.  Summary: Licensee did not contest the charge of, in the State of California, failing to accurately document the administration or wastage of the controlled substance fentanyl for three patients.

The Board of Regents voted on March 13, 2023, to deny the application for restoration of the physician license of Conrad R.Williams, Roselle, NJ 07203.  Dr. Williams’ license was surrendered April 9, 2015.
