Consolidated State Performance Report Data Reporting Form
2024 CSPR Data Collection Tool (2022-23 data)
Background:
The New York State Education Department (NYSED) is required to annually collect data for the completion of the Consolidated State Performance Report (CSPR) submission to the United States Department of Education. The CSPR form must be completed for each neglected or delinquent facility under your corporate umbrella that receives Title I, Part D funds in New York State. Agencies/institutions with multiple sites must submit one collection form for each site. The reporting period to be used on the collection form must be July 1, 2022 through June 30, 2023.
Instructions:
The 2024 CSPR data is being collected through the 2024 CSPR Data Collection Tool. When you open the file, you may see a security warning. Click on the “Enable Content” box in the yellow warning line. The form has two tabs. The first tab is the data collection tool where you will input your facility data. The second tab (Notes) is a guide regarding the data collection methodology. Do not enter facility data on the Notes tab.
NOTE: There have been changes this year to several sections of the 2024 CSPR Data Collection Tool. These changes have been outlined in the document entitled “Changes to 2024 CSPR Data Collection Form.” When completing the form, please make sure that you are using the latest version.
Please ensure that the workbook is filled out completely. Each indicator must be addressed. If the indicator does not apply, put a zero in the workbook for that line. Where necessary, also put a comment in the comments section (see below).
Neglected facilities may not have achievement data available, therefore, basic facility information and student demographic data should be submitted. If a variable is not applicable for a facility, please indicate in the comments section why it is not applicable e.g.:

Once the form is complete save the file with the following naming convention: CSPR2024-[INSERT FACILITY NAME]. For example, CSPR2024-Safe Space Children’s Services. The form can be emailed to nd@nysed.gov. The completed file is due to NYSED by close of business, Thursday, October 5, 2023.
